If you are currently attending or have recently completed your Phase III Cardiac Rehabilitation program you will receive details of Phase IV programs that are available to you and run locally. For the most part, people who attend Phase IV classes have transferred across following their completion of a NHS run Phase III eight week program (which I assist in Cambs) and/or via a GP referral.
